admission criteria
To be admitted to Promise of Hope, you must meet the criteria listed below.
You can fill out the admission application online, once you submit your initial application you will be notified via email that it has been received along with further instructions to complete. You may also click the link for the printable version of the application below and mail that to Promise of Hope or fax to (478) 676-4675.
- Primary diagnosis of chemical dependency.
- No violent tendencies.
- No dual diagnosis.
- Adequately de-toxed (considered case by case).
- Psychological diagnosis (considered on a case by case basis).
- No medical conditions that would prohibit client from complying with program.
- All legal matters in order.
- Client must be willing to give Promise of Hope consent to talk with probation/parole officer.
- In questionable cases, admission would be based upon interview with the Operational Team and client would be responsible for transportation to meeting.
- Sincere desire and willingness to be in program.
- Seeking placement for self and not for family/significant other.
- 18 years of age or older.
